First Visit

Your visit will be scheduled according to the anatomical nature of the tooth on which treatment is needed. For example, anterior (front) and bicuspid teeth are scheduled for 30-minute appointments, but more time is allowed for the treatment of molars.

When you arrive at our office, we will ask you to fill out several forms, including a general information form, a dental insurance form, a financial agreement and an endodontic consent form.

When seated, one of our highly trained team members will take a comprehensive history of the tooth’s symptoms and a radiograph. Your doctor will perform a thorough examination of the tooth in question and its surrounding area. He will then consult with you about the tooth’s prognosis and treatment options. We will attempt to complete your endodontic treatment in one visit; however, conditions in the tooth may require us to schedule a completion appointment.

Please assist us by providing the following information at the time of your consultation:

  • A list of medications you are presently taking.
  • Dental insurance information, including the subscriber’s social security number and date of birth.

    IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

  • Please alert the office if you are required to premedicate with an antibiotic prior to dental treatment or if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.).

X-Rays

We use the latest in digital technology for your radiographs, therefore it is not necessary for you to bring radiographs from your referring dentist. We take only the radiographs needed and archive them for future reference.
